2. What’s the Difference Between Semaglutide and Tirzepatide? Injections and Pills?

While semaglutide (Wegovy) and tirzepatide (Zepbound) are often grouped together, they work somewhat differently within the body. To understand the difference, it helps to understand how these medications work in the first place.

GLP-1 receptor agonists mimic naturally occurring hormones released by the gastrointestinal tract after eating. These hormones communicate with multiple systems throughout the body, including the pancreas, stomach, and brain.

Their effects include:

  • Slowing gastric emptying
  • Increasing feelings of fullness
  • Reducing hunger signals
  • Improving insulin secretion
  • Reducing glucagon production
  • Supporting better blood sugar regulation and metabolic health

Semaglutide

Semaglutide activates the GLP-1 receptor and is available as FDA-approved branded medications, including Wegovy® and Ozempic®.

Clinical trials have demonstrated significant weight loss (15%-19%), improved blood sugar control, and reduced cardiovascular risk factors.

Tirzepatide

Tirzepatide takes this concept a step further. In addition to activating GLP-1 receptors, tirzepatide also activates GIP (glucose-dependent insulinotropic polypeptide) receptors.

Researchers believe this dual-receptor activity contributes to enhanced appetite control, improved insulin sensitivity, and the greater average weight loss (15%-21%) observed in clinical studies.

Tirzepatide is available through medications such as Zepbound® and Mounjaro®.

What About the New Weight Loss Pills?

The latest development in medical weight loss is the introduction of oral GLP-1 medications. These offer our clients another option beyond weekly injections. However, it’s important to understand that not all weight loss pills work the same way.

One of the new oral options is the Wegovy® pill, which contains the same active peptide ingredient found in the Wegovy (semaglutide) injection. Because the medication contains the same peptide, it must be taken under very specific conditions to ensure proper absorption. Clients must take it first thing in the morning on an empty stomach and avoid eating or drinking for approximately 45 minutes afterward.

The benefit? Clinical studies suggest the Wegovy pill may produce weight-loss results similar to the injectable version, with many patients experiencing an average weight loss of 15-19% of their body weight.

Another oral option is Orforglipron (expected to be marketed as Foundayo® by Eli Lilly). Unlike semaglutide, this medication is a small-molecule GLP-1 receptor agonist rather than a peptide-based medication. Because it does not contain a peptide, it does not require the same strict dosing instructions as oral semaglutide and offers greater flexibility for many clients.

Current data suggest an average weight loss of approximately 7.5-11% of body weight with Foundayo.

To put these options into perspective:

Foundayo® (oral small-molecule GLP-1):
Average weight loss of approximately 11% of body weight

Wegovy® Injection and Wegovy® Pill (semaglutide):
Average weight loss of approximately 15-19% of body weight with injection and 14% with pill

Zepbound® (tirzepatide):
Average weight loss of approximately 15-21% of body weight

3. How Much Weight Can I Lose on a GLP-1?

The answer depends on a variety of factors, including the prescribed medication, your starting weight, underlying metabolic health, adherence to treatment, dietary habits, physical activity, sleep quality, and stress levels.

That said, clinical studies provide some helpful benchmarks.

Semaglutide (Wegovy®)

Clients receiving weekly injections of semaglutide have demonstrated an average weight loss of 15%-19% of their starting body weight when combined with lifestyle modifications (14% with oral administration). For someone weighing 250 pounds, that could translate to roughly 35-40 pounds of weight loss in one year or less.

Tirzepatide (Zepbound®)

Tirzepatide has produced some of the most impressive results ever seen in obesity medicine, with studies demonstrating average weight loss of approximately 15%-21% of body weight. For many clients, this begins to approach the weight loss traditionally associated with certain bariatric procedures.

Oral GLP-1 Medications

While oral options continue to evolve, they generally produce more modest weight loss compared to injectable medications. For example, those taking the Wegovy® pill can anticipate a 14% overall weight loss and 11% with Foundayo. However, they may still provide meaningful benefits for clients who prioritize convenience or prefer to avoid injections.

But here’s something that often gets overlooked:

Weight loss and fat loss are not the same thing.

That’s why I rarely view success as reaching a goal weight and then stopping treatment altogether. More often, we transition into a maintenance strategy by reducing the dose, extending the dosing schedule, or adjusting the plan based on the individual’s needs. My goal isn’t to keep someone on the highest dose forever—it’s to find the lowest level of support needed to help them maintain their results and continue feeling their best.
